Program Priority: Increase awareness of health issues related to underserved populations.

BridgeToHealthLogo600The Bridge to Health Survey has been a valued source of information on the health status of the adults in the Northeastern Minnesota / Northwestern Wisconsin region over the past 15 years.  The survey was first conducted in 1995 and repeated in 2000, 2005, and 2010.

Generations Health Care Initiatives has directed and been the major sponsor of the 2005 and 2010  Bridge to Health Surveys.  Throughout its history, this Survey has been a collaborative effort involving Over 50 sponsors.

Nearly 4500 adults from a nine county area participated in the survey.  Data was collected via  telephone interviews.

The Survey topics include:

  •     o Overall health status    
  •     o Disease prevalence (physical and mental health)
  •     o Health risk behavior (tobacco use, alcohol use, diet, exercise, etc.)
  •     o Access to health and dental care
  •     o Preventive Health Screenings

Survey results are available for the region, individual counties, and the City of Duluth.
